Naturally, if your home care needs change, your Client Relationship Manager can schedule meetings with your caregiver and Registered Nurse or coordinate a referral to a home health agency.

The best time to start planning your transition home is while you are still in the hospital or rehab facility.

Therefore, when you contact us, we will send one of our specially trained Community Liaisons to you to discuss your situation and create a plan to transition you back home.

When your discharge day comes, we can be with you to assist with transportation home and make sure we account for all your possessions. Our Transitional Services caregivers are vetted as safe drivers and are fully insured with our company insurance.

This ongoing communication helps family members stay informed on their loved one’s well-being and ensures that any changes in their condition are addressed promptly.

Following your Needs Review, one of our Registered Nurses will complete a detailed nursing assessment. This evaluation is among the most important steps in creating your personalized care plan — the foundation of your home healthcare services.

Once your services begin, our Registered Nurses will continue visiting you on a scheduled basis. During these visits, they will complete follow-up evaluations and document any changes observed since the previous visit.

If our Nurse identifies new concerns or changes in your condition, our Nurse will provide guidance and additional training. Should your condition change significantly, our Nurse will update the care plan and coordinate directly with your caregiver.

Severn Healthcare Personal Care’s Registered Nurses also communicate with your family members and other healthcare professionals involved in your care while providing referrals to additional providers or specialists when necessary.
